Participant Waiver

The undersigned participant and participant’s parents or legal guardian represent that they are fully aware and apprised that there are risks and physical demands incident to the physical activities involved in dance completion and dance training.  Participant and guardian further represent, having full knowledge of participant is physically fit to participate in dance activities.  Participants and guardian’s and participant’s heirs, assigns, executors, and representatives, hereby release imdemnify and hold harmless L.A. Dance (Lora’s Academy of Dance). Lora Wood, Vistancia Community, Shea Homes or CMT, and each of their affiliates, officers, agents for any claims, demands, losses, actions, liability, costs, damages, and expenses omission related in a way dance activities or operation of Lora Wood or L.A. Dance. Participant and Guardian acknowledge that Lora Wood, Lora’s Academy of Dance, or the school which dance classes are offered, DOES NOT provide insurance for injuries suffered in connection with dance activities, and it is the participant and guardian to obtain adequate insurance coverage to cover the participant’s risk of injury. Tuition is due every four weeks. Any tuition that is not received the week that tuition is due will have a $15 late fee applied.  All fees must be paid in full prior to receiving recital costume and/or participating in any dance activities. There is a $25 fee for any and all returned items/checks.
